MIS Model Questionnaires

The Malaria Indicator Survey contains three core survey instruments – the Household Questionnaire, Biomarker Questionnaire, and the Woman’s Questionnaire.

Household Questionnaire

The Household Questionnaire contains basic demographic information, a complete listing of household residents and visitors, a record of selected household assets, and the recommended net roster for recording insecticide-treated net (ITN) possession and use.

Biomarker Questionnaire

The Biomarker Questionnaire records hemoglobin measurements for assessing anemia status and malaria parasitemia measurements for assessing malaria infection in children age 6-59 months.

Woman's Questionnaire

The Woman’s Questionnaire includes information on characteristics for women age 15-49 as well as questions for recording a recent birth history, pregnancy status, and use of intermittent preventive treatment with sulfadoxine-pyrimethamine (SP) during pregnancy. The Woman’s Questionnaire is also designed to collect information from each woman interviewed about her children under age five. This includes information on care seeking, diagnostic testing, and access totreatment with antimalarials for children with reported fever.
